Patient Choice - Your Views Count

NHS Cornwall and the Isles of Scilly want to ensure the community and mental health services we commission meet the needs of patients and carers living in Cornwall and the Isles of Scilly.

Increasing patients' choice of provider can empower patients and carers, help drive up quality, encourage service innovation, improve access and address gaps.

Changes are being introduced by the government to give patients more choice about the services they receive and more choice about who provides those services.

NHS Cornwall and Isles of Scilly will start by identifying three community and mental health services for extending choice of provider by September 2012.  In order to help us identify the most appropriate community based services, we are asking the local population, and users of these services, to identify which they feel would most benefit from an increased choice of provider.

We would appreciate your help in encouraging your service users to complete either the online questionnaire or the attached PDF which has a Freepost address for return when completed.

Online questionnaire link:  http://www.surveymonkey.com/s/X3TT8GS

If you wish to receive the questionnaire in an alternative format, please contact the Patient Advice and Liaison Service who will do their best to meet your requirements.
